Tuesday, March 13, 2012 Wapakoneta Elementary 12:00-8:00
Wednesday, March 14, 2012 Wapakoneta Elementary 8:00-4:00
Thursday, March 15, 2012 Cridersville Elementary 10:00-6:00
Friday, March 16, 2012 Cridersville Elementary 8:00-4:00
To register, you MUST bring:
1. Your child's birth certificate
2. You child's social security card
3. Your child's immunization records (immunization requirement information)
4. Proof of residency (utility bill, deed, rental lease, etc.) showing your name and current address
5. Custody papers (if applicable)
An appointment for your child's kindergarten screening will be made at the time of registration. Screening will be held at Wapakoneta Elementary on July 24 and 25, and at Cridersville Elementary on July 26.
Your child must be 5 years old on or before August 1, 2012, to be able to enter kindergarten in the fall of 2012.
